1. Overview
Divine Heritage Services LLC (“Divine Heritage,” “we,” “us,” or “our”) is committed to protecting the privacy of the individuals and families we serve. This Privacy Policy explains how we collect, use, share, and safeguard information when you visit our website, contact us, or receive care through our practice.
Because we provide mental health services, much of the information we collect is Protected Health Information (PHI) governed by the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws. PHI is governed primarily by our Notice of Privacy Practices, which is provided to every patient and is available on request.
2. Information We Collect
We collect information that you provide to us directly, information that is created in the course of providing care, and limited information collected automatically when you visit our website.
Information you provide directly includes your name, contact details, demographic information, insurance information, the reason you are reaching out, and anything you share in messages, intake forms, or scheduled visits.
Information created during care includes clinical notes, diagnoses, treatment plans, medications, lab or assessment results, and any other information generated by our clinicians as part of your care. This information is Protected Health Information.
Information collected automatically includes basic technical data such as IP address, browser type, pages viewed, referring URL, and similar log information. We do not use third-party advertising trackers on this site.
3. How We Use Your Information
We use your information to provide and improve mental health care, to communicate with you, to coordinate care with other providers (with your authorization), to bill for services, to comply with legal and regulatory obligations, and to operate and secure our website.
We do not sell your personal information or PHI. We do not use your PHI for marketing without your written authorization, except for limited communications expressly permitted by HIPAA (for example, appointment reminders).
5. How We Protect Your Information
We use administrative, technical, and physical safeguards designed to protect your information against unauthorized access, disclosure, alteration, and destruction. These include encryption in transit, access controls, workforce training, audit logging, and HIPAA-compliant infrastructure for systems that handle PHI.
No method of transmission over the internet is completely secure. We continuously review and improve our safeguards, but we cannot guarantee absolute security.
6. Your Rights
Under HIPAA, you have the right to inspect and obtain a copy of your PHI, to request a correction, to request restrictions on certain uses or disclosures, to request confidential communications, to receive an accounting of certain disclosures, and to receive a paper copy of our Notice of Privacy Practices on request.
Depending on where you live, you may have additional rights under state law (for example, California, Colorado, Virginia, and similar privacy statutes), including the right to know what personal information we hold about you, to request deletion, to opt out of certain processing, and to be free from discrimination for exercising your rights. To exercise any of these rights, contact us using the information at the end of this policy.
8. Children’s Privacy
We provide care to adolescents in addition to adults, and information about minor patients is treated as PHI under HIPAA and applicable state law. Our website is not directed at children under 13, and we do not knowingly collect personal information from children under 13 outside of the clinical care context.
9. Third-Party Links and Services
Our site may contain links to third-party websites, such as crisis resources or insurance plan portals. We are not responsible for the privacy practices of those sites, and we encourage you to review their privacy policies before sharing information with them.
10. How Long We Keep Information
We retain PHI for the period required by federal and state law, which is typically several years after the last date of service. We retain other personal information for as long as needed to fulfill the purposes described in this policy, to comply with our legal obligations, and to resolve disputes.
11. Changes to This Policy
We may update this Privacy Policy from time to time. When we do, we will update the “Last updated” date at the top of this page, and significant changes will be highlighted on the site or communicated to active patients.
